Description
The Healthy Families program of Family and Children’s Center is a comprehensive and intensive home visitation program that has a full-time opening for a Case Manager to work with a supportive team and establish a trusting relationship with overburdened families by delivering service that is culturally competent, strength-based, family centered and family directed.
Responsibilities Include:
• Meet with families in the least restrictive environment, typically their homes or in the community in Vernon County
• Assist parents in improving their skills so as to optimize a positive home environment
• Increase the family’s ability to problem-solve and advocate for themselves and their children
• Identify and refer families to additional community resources, including healthcare and other basic necessities
• Work cooperatively with families to establish goals and develop a plan for accomplishing those goals
• Conduct regular assessments of the growth and development of the children participating in the Healthy Families program
• Suggest, lead, and model parent/child activities
• Maintain written documentation of all activities and contact with families, as well as referrals or contacts made on behalf of families
• Participates in all agency staff meetings, team meetings, and other meetings as assigned
Requirements
- Minimum of a bachelor’s degree in human services or related field
- 3 years experience after bachelor’s degree including case management experience
- Working knowledge of prenatal and child development, mental health and community resources in order to provide support to young families
- Access to personal vehicle
- Valid driver’s license and vehicle insurance
- Good driving record
The Day Treatment Clinical Trainee will be responsible for leading groups for children, working collaboratively with a strong team, and providing individual services to clients in the group and their families.Appropriate candidates can be a Clinical Trainee or full licensed therapist. Clinical supervision is provided at no cost and is considered part of the weekly work schedule. Clinical consultation or supervision for a variety of modalities is available and encouraged to grow a provider’s expertise.

Requirements:
The minimum requirements are a master’s degree with Clinical Trainee status (clinical supervision will be provided) or Mental Health Professional licensure (current or eligible) in the state of Minnesota. Grant funding for training in evidence based practices approved for early childhood clients may be available.
Case Manager- Stepping Stones
Responsibilities Include:
- Coordinate and provide services to non-offending caregivers and their children
- Act as a vital member of the team from the initial investigation on through case disposition or until services are no longer needed
- Forensic interviewing
- MDT coordination
- Providing assessments
- Crisis intervention
- Information and referrals
- Accompanying children and families to court
- Public education
- Data collection and program evaluation
- Acting as a liaison between the system at large and support fundraising/ special events
Requirements
- Minimum of a Bachelor’s degree in social work or human services related field
- At least six months of professional experience working with children and families affected by abuse
- Experience or knowledge in the areas of child development, crisis intervention, case management, and child abuse investigations are preferred.
