Doula – HUB

As a Community Health Worker (CHW)/Doula, you help to bridge the gap between health care delivery and the social service sector for cost savings, improved population health outcomes, and increased client experience and engagement. Through community care coordination, a CHW will provide social, behavioral, and resource support to clients as they navigate the health care and social service systems. In addition, you will provide physical, emotional, and educational support for women throughout the birthing process (prenatal/labor/postpartum). The CHW/doula works in partnership between Coulee Health and Great Rivers HUB, and this position will utilize the evidence-based Pathways Community HUB to bridge the gap between healthcare delivery and social services.

Position Duties:

  • Connect clients to health care/social service resources and help client navigate those resources.
  • Facilitate communication and client empowerment in interactions with healthcare/social service system
  • Provide education directly related to prenatal, birthing and postpartum
  • Educate clients to help understand their health condition(s) and develop strategies to improve their health and well-being.
  • Document in a timely and accurate manner all encounters, referrals, and follow ups in the CCS electronic database.
  • Build strong relationships with other area public health professionals and health providers to create opportunities for collaboration and program outreach.
  • Perform monthly home visits to clients
  • Help to build understanding and social capital to support healthier behaviors and lifestyle choices.
  • Attend required Family & Children’s Center meetings and Great Rivers HUB meetings as appropriate

This is a community based role, consisting of both work time in the office and in community settings. Travel within the community is required. Because the doula is expected to attend childbirth, some nights and weekends may be required and time can be flexed to meet this need.

Education/Training/Experience Qualifications

  • High School Diploma/GED or equivalent
  • Willing to obtain both Community Health Worker Certification and Doula certification if not already certified. Training will be provided through agency.
    • Upon hire and completion of CHW Certification, one should be able to demonstrate an understanding of (CHW) health care delivery systems and the Pathways community HUB model.
  • 1-2 years of experience/knowledge with the following communities:
    • Low-income individuals/families
    • Pregnant Women
    • Families
    • Culturally & ethnically diverse populations
    • Individuals managing chronic diseases including cardiovascular disease and type 2 diabetes
  • Knowledge of local resource agencies and resources available

 

 

Case Manager – Independent Living

A Case Manager needed to work on independent living skills with youth that are in or have been in out-of-home-care in a 15 county region.

This position will cover the following 8 counties: Buffalo, Chippewa, Clark, Dunn, Eau Claire, Pepin, Pierce, and St. Croix.

While this position reports to the supervisor in our La Crosse office, this is a remote position in that it provides in person services to clients in the above counties.

Please apply if you live in one the counties listed above, as the Independent Living Specialist will be driving throughout the 8 county range listed above to provide in person services to clients.

 

Responsibilities include conducting assessments, groups, and one-on-one services with the youth, as well as attending team meetings. The Case Manager works closely with and supervises interns.

Applicants must have a Bachelor’s Degree in Social Work and three years’ experience with at-risk youth. Must have the ability to seek out and collaborate with community resources within each of the assigned 7-8 counties. The Case Manager must have a valid driver’s license and access to a reliable vehicle for travel.
