Healthy Families Case Manager, Viroqua – PART TIME

The Healthy Families program of Family and Children’s Center is a comprehensive and intensive home visitation program that has a part-time opening for a Case Manager to work with a supportive team and establish a trusting relationship with overburdened families by delivering service that is culturally competent, strength-based, family centered and family directed.

Responsibilities Include:

• Meet with families in the least restrictive environment, typically their homes or in the community in Vernon County

• Assist parents in improving their skills to optimize a positive home environment

• Increase the family’s ability to problem-solve and advocate for themselves and their children

• Identify and refer families to additional community resources, including healthcare and other basic necessities

• Work cooperatively with families to establish goals and develop a plan for accomplishing those goals

• Conduct regular assessments of the growth and development of the children participating in the Healthy Families program

• Suggest, lead, and model parent/child activities

• Maintain written documentation of all activities and contact with families, as well as referrals or contacts made on behalf of families

• Participates in all agency staff meetings, team meetings, and other meetings as assigned

Requirements

  • Minimum of a bachelor’s degree in human services or related field
  • 3 years experience after bachelor’s degree including case management experience preferred
  • Working knowledge of prenatal and child development, mental health and community resources in order to provide support to young families
  • Access to personal vehicle
  • Valid driver’s license and vehicle insurance
  • Good driving record

 

 

Mental Health Practitioner, ARMHS – FULL TIME

Responsibilities include:

  • Assessing functionality
  • Developing rehabilitative treatment plans
  • Maintaining client file documentation in accordance with DHS licensing standards
  • Crisis prevention and de-escalation
  • Medication monitoring
  • Working with residents in their home toward goals in mental health recovery in-group and individual formats
  • Some weekend and on-call hours may be included.

Requirements

  • Bachelor’s degree in a human services field AND at least 2000 hours of supervised experience with services to persons with mental illness
  • strong computer skills
  • Be a team player
  • Have good interpersonal skills
  • Emphasis on dual diagnosis treatment recommended
