Direct Care Staff, Day Treatment- PT
Responsibilities Include:
- Work collaboratively with Day Treatment staff on duties that are necessary for program implementation
- Participate as a member of the Day Treatment team including
- Provide transportation for Day Treatment clients
- Complete daily documentation
- Use appropriate behaviors and positive communication skills when interacting with members in the community as part of the Day Treatment team.
Requirements:
- Must be 20 years of age or older
- Must have a good driving record/be insurable
- Minimum of high school diploma or equivalent
Mental Health Practitioner, Day Treatment- PT
Responsibilities Include:
- Assessing functionality
- Developing rehabilitative treatment plans
- Maintaining client file documentation in accordance with DHS licensing standards
- Crisis prevention and de-escalation
- Medication monitoring
- Working with residents toward goals in mental health recovery in group and individual formats
- Position requires on-call hours.
Requirements:
- Must qualify as a Mental Health Practitioner by having a bachelor’s degree in a human services field and at least 2000 hours of supervised experience with services to persons with mental illness, or 6,000 hours of such experience.
Case Manager, Day Treatment- PT
Responsibilities Include:
- Case management
- Coordinating treatment planning & documentation
- Overseeing daily operations
- Skills group facilitation
