Case Manager – Supported employment
Responsibilities include:
- Assessment and treatment planning
- Assisting clients to attain resources
- Meet with local employers regarding employment
- Provide skills training and transportation
Requirements
- Bachelor degree in human services, social work, psychology, sociology or a related field
- Ability to work positively with persons with mental health diagnoses
- Must have good driving record, access to personal vehicle
- Willing to travel across multiple counties
