Case Manager – Healthy Families

The Healthy Families program of Family and Children’s Center is a comprehensive and intensive home visitation program that has 2 part time openings for a Case Manager position to work with a supportive team and establish a trusting relationship with overburdened families by delivering service that is culturally competent, strength-based, family centered and family directed.

Responsibilities Include:
• Meet with families in the least restrictive environment, typically their homes or in the community in La Crosse County
• Assist parents in improving their skills so as to optimize a positive home environment
• Increase the family’s ability to problem-solve and advocate for themselves and their children
• Identify and refer families to additional community resources, including healthcare and other basic necessities

Requirements

  • Minimum of a bachelor’s degree in human services or related field
  • 3 years experience after bachelor’s degree including case management experience
  • Working knowledge of prenatal and child development, mental health and community resources in order to provide support to young families
  • Access to personal vehicle
  • Valid driver’s license and vehicle insurance
  • Good driving record

 

Case Manager  – Independent Living Specialist

As an Independent Living Specialist, you will focus on resource and skill development to help young adults live successfully in the community by building an Individualized Life Skills Plan.

This position will cover the following 8 counties: St. Croix, Pierce, Dunn, Pepin, Buffalo, Chippewa, Eau Claire, Clark

While this position reports to the supervisor in our La Crosse office, this is a semi-remote position in that it provides in person services to clients in the above counties.

Please apply if you live in one the counties listed above, as the Independent Living Specialist will be driving throughout the 8 county range listed above to provide in person services to clients.

Responsibilities include:

  • Case Management of youth within the following counties: St. Croix, Pierce, Dunn, Pepin, Buffalo, Chippewa, Eau Claire, Clark
  • Conducting assessments
  • Managing groups
  • Providing one-on-one services with the youth, including assistance with:
  • College applications
  • Job searches
  • Budget management
  • Obtaining affordable housing
  • Meal planning
  • Attending team meetings.
  • Works closely with and supervises interns.

Requirements include:

  • Bachelor’s Degree
  • Experience working with adults with mental health needs.
  • Candidates must be familiar with the Human Service system, recovery principles, and appropriate ethics and boundaries.
  • Have access to a personal vehicle
  • Multi-county travel
